Fall Prevention Program

SSM Health at Home provides comprehensive care for patients who are high risk for falling. Per the CDC (Centers for Disease Control and Prevention), one out of three older people fall each year, but less than half tell their doctor. Chances of falling doubles once you have already taken a fall.

Risk factors for falling include:

  • Muscle weakness
  • Poor balance
  • Drop in blood pressure
  • Foot problems
  • Vision issues
  • High number of medications
  • High risk medications
  • Sensory or sensation issues
  • Memory or cognitive issues
  • Recent illness or surgery
  • Previous fall
  • Environment - obstacles, lack of safety equipment

With our fall prevention program, you will receive care by highly trained and experienced staff including registered nurses, physical therapists, occupational therapists, speech therapists and social workers. You will undergo a comprehensive fall risk assessment, indentifying a treatment plan specific to your needs. We work closely with your doctor to determine the best course of care.
