SSM Health at Home’s telehealth service helps congestive heart failure patients remain in their homes during recovery, rather than face lengthy stays in, or readmission to, the hospital.

How does our telehealth service work?

It all starts with a treatment plan created by your physician and your SSM Health at Home nurse. To support that plan, SSM Health at Home installs a telemonitor in your home for 30 to 60 days at no cost to you or your insurance provider. An SSM Health at Home technician brings the equipment to you and provides instruction on its use. You then record your blood pressure, weight and oxygen saturation (the oxygen levels in your blood) using the equipment provided. That data is transmitted via telephone line to SSM Health at Home's experienced cardiac nurses who review the results daily and immediately report any concerns to you and your physician.

Keeping you out of the hospital

Not only are you able to monitor, recognize and manage your own symptoms using the telemonitors, you can help decrease your chances of being readmitted to the hospital after using our service. In some locations, we also offer telehealth for monitoring chronic obstructive pulmonary disease (COPD), acute myocardial infarction and pneumonia. You must have a physician order to receive SSM’s home health services.
