We need to know if your condition changes or you are experiencing new symptoms. Please do not hesitate to contact SSM Health at Home should you have a problem or concern. A registered nurse is available by telephone 24 hours a day, seven days a week for emergency calls. The nurse will advise you over the telephone or send a member of our team to visit your home if necessary.

What to expect if you call between 8 am to 4:30 pm:
You will reach the SSM Health at Home office. You’ll be able to speak to a supervisor who will address your request or issue.

What to expect if you call after hours:
A registered nurse is available for emergency calls 24/7. Call the number your nurse has given you; your call will be directed to our after-hours call center and a nurse will respond. If you do not receive a response within a reasonable timeframe, please call again.

What to expect if you call between 8 am to 4:30 pm:
You will reach the SSM Health at Home Hospice office. If your nurse is in the office, you will be able to speak with him or her directly. If your nurse is out visiting another patient, we will call that nurse, and he or she will return your call.

What to expect if you call after hours:
A registered nurse is available by telephone 24 hours a day, seven days a week. Your call will be directed to our after-hours call center and a nurse will respond within 15 minutes. If you do not receive a response within that time frame, please call again. The nurse will advise you over the telephone --or send a member of our team to visit your home if necessary.
