We Are Here for You & Your Family

Our care teams focus on improving quality of life, providing not only physical care but also emotional and spiritual support. We work with your health care provider to create an individualized care plan for each patient. We also support caregivers and loved ones both during and after the hospice journey.

Most hospice patients prefer to remain at home and our hospice team helps make that happen. Care can be provided in the patient’s home, a family member’s home, a nursing home, assisted living, or other long-term care facility. In Wisconsin, care can also be provided at one of SSM Health’s residential care facilities for those patients who are unable to remain at home.

Your Hospice Team

You are the focus of our care. Your care team will help you identify goals and work to support your needs physically, emotionally, and spiritually. We are here to help you experience the best quality of life possible, surrounded by the people and things you love.

Your family will provide care for you with the support of the SSM Health at Home hospice team. We are here for them throughout every phase of the hospice journey.

Your physician certifies you are eligible for service and continues to direct your care while you are receiving hospice services.

The hospice medical director co-certifies your eligibility and leads the hospice team in developing a plan of care unique to you and your needs.

Your nurse will work with the team to develop and coordinate your care. They will ensure your symptoms are controlled and your needs are met. Your nurse will also provide support and education for you and your caregivers.

Your home health aide will provide personal care for you. They will also lend emotional support to you and your family.

The medical social worker will meet with you and your family to develop a plan to meet your emotional, social, spiritual, and financial needs. They can provide counseling and direct patients and families to available resources.

Your spiritual and bereavement counselor is valuable resource for you and your family. They can provide counseling and support as well as assist with memorial preparation. They will support your spiritual needs throughout your journey and continue to support your loved ones for more than a year.

You can request hospice volunteers to visit with you or provide a brief respite for your family.
